Referral Education Assistance & Prevention Program (REAP)

Helping older adults live a safe and satisfying life.

The need for behavioral health support doesn’t end as you age! We all deserve to live a fulfilled life. You can talk to REAP counselors about any concerns regarding stress, relationships, grief, emotional wellbeing, substance misuse, family dynamics or other issues that are affecting your lifestyle quality of life!

What is REAP?

REAP provides community-based, short-term support for older adults who live in New Hampshire, caregivers and family members of older adults, and professionals who work with and for older adults.  The team’s goal is to address concerns around mental health, substance use and cognitive functioning.

We administer this program statewide and contract with the other nine New Hampshire community mental health centers to carry out this service throughout the state.

  • Reap counselors provide with older adults and their caregivers with:

    • Brief Counseling
    • Technical Assistance (to housing and other professionals who serve older adults)
    • Education Programs for older adults and professionals who work with older adults
    • NH residents aged 60+ and anyone living in Senior Housing,
    • Caregivers and family members of anyone eligible for REAP, and
    • Professionals who work with and / or on behalf of older adults in NH.

    Exceptions: Permanent residents of long-term care facilities and active clients of a NH Community Mental Health Centers are not eligible for the REAP Program.
