We offer accessible mental health counseling and supportive services aimed at empowering your whole life.

Services Offered

We offer a wide array of compassionate, comprehensive therapy and support services to adults 18 and older. Our approach is evidence-based, positive, and goal-directed.  Our services are available at our offices, in your home, at work and at other community locations.

Following a thorough evaluation, we’ll develop an individualized treatment plan with you. Our aim is to meet you where you are and help you identify and resolve issues that are keeping you from successfully achieving your goals.

  • The power of relationships is important in recovery and in our daily lives. Through individual, couples, family, and group therapy, we provide safe spaces where people can truly feel heard.

    Available at both offices, in your home, via telehealth, and specified co-located sites in the community, we provide therapy for various issues: anxiety, depression, mood disorders, relationships, trauma, etc.

    We have different specialties and areas of expertise to meet your needs. Our clinical specialties include Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), Eye Movement Desensitization & Reprocessing Therapy, Motivational Interviewing, Reprocessing Therapy and Solution Focused Therapy.

    After assessment and review, your clinician and you will develop a treatment and care plan which may include some of the additional services we offer, as noted below.

  • Our integrated clinical team manages small caseloads of adults experiencing severe and persistent mental illness enabling these individuals to live independently in the community. ACT clients often have needs that have not been effectively addressed by traditional, less intensive services. Team members work with clients in their homes, work setting, or places in the community on a 24/7 basis.

  • Critical Time Intervention (CTI) is an intensive 9 month case management model designed to assist adults age 18 years and older with mental illness who are going through critical transitions and who have functional impairments which preclude them from managing their transitional needs adequately. CTI promotes a focus on recovery, psychiatric rehabilitation and bridges the gap between institutional living and community services. CTI differs from traditional case management because it is time limited, focused and follows a three phased approach.

  • It is estimated that three (3) in 100 people will experience a psychotic episode at one point in their lives. There are early intervention techniques that provide the ability to experience a rewarding and meaningful life. FEP measurable goals may include tracking hospitalization frequency, recurrence of relapse, length of treatment, stable employment and housing, medication management, and peer support involvement. We offer FEP services for clients ages 15-35.

  • FSS are face-to-face clinical interventions provided in the individual’s home or community. These services are designed to enhance resiliency, recovery, and integration in the community. There are four distinct Functional Support Services: Crisis Support Services, Family Support Services, Medication Support Services, and Therapeutic Behavioral Supports.

    We utilize the NAVIGATE Program which is aimed at helping clients (and their family members) negotiate their through the complexities around mental illness, through collaboration with a small and committed treatment team. We set goals with the client towards regaining control over their lives and achieving their personal goals. the NAVIGATE Program involves education around medication treatment, family education, individualized resiliency training and employment/educational work,

  • We offer a variety of housing and housing support options:

    • Fairweather Lodge is a beautiful eight-bed community residence in Greenland, NH for adults with severe and persistent mental illness. We provide staffing and on-site care and support 24/7.
    • Our Housing Specialists work with clients to access the NH Housing Bridge Subsidy Program, which provides housing vouchers until a permanent housing voucher becomes available.
    • Our Projects for Assistance in Transition from Homelessness (PATH) program connects such persons experiencing homelessness with services in the community: Emergency Food Stamps, Medicaid, Social Security benefits, mental health care, and physical care, etc. We manage ongoing follow-up, as well as additional services including referrals for primary care, job training, education, transportation, and relevant housing services.
  • IMR helps you facilitate your life goals beyond your challenge or illness. We couple education, support, and encouragement with your active goal setting and participation. This partnership embraces the concepts of recovery, hope, and personal empowerment.

  • Your physical health is a critical support for your mental health. Our InSHAPE program is an innovative wellness program aimed at improving the physical health and the quality of life for individuals with severe mental illness through promotion of exercise, wellbeing education and a healthy diet.

    The program offers:

    • Personal health mentors
    • Access to fitness activities provided by local program partners
    • Nutrition counseling and education
    • Smoking cessation support
    • Medical liaison support
    • Support for receiving regular medical check-ups and active management of chronic health conditions
  • Choosing to get sober and into recovery takes courage! Our outpatient SUD program and recovery specialists offer you support by focusing on skills in early recovery and relapse prevention skills/tools. Our services include:

    • Assessment for SUD or Integrated Treatment for Co-Occurring Disorders (ITCOD)
    • Individual, group and psychopharmacological counseling
    • Medication Assisted Treatment (MAT)
    • Smoking Cessation services
    • Certified Recovery Support services
  • IPS-SE has shown that employment offers meaning to a person’s life and plays a significant role in the process of recovering from mental health challenges.

    Our vocational specialists partner with you throughout your employment and educational journey. Together, you will identify your skills, experiences and interests and match those with competitive employment and vocational educational opportunities in the community.

    Our specialists can help you in obtaining accurate information about how working may affect benefit programs such as Social Security and Medicaid and discuss your options.

  • People with dual diagnoses of mental health disorders and substance use disorders have “co-occurring disorders” and they need to be treated in an integrated manner for recovery. Co-occurring disorders are extremely common for people suffering with addiction. Our specialized approach involves Integrated Recovery Plans to support you throughout your recovery goals.

  • Mental Health Court Services

    Mental Health Court services are provided to non-violent offenders whose offense is believed to be caused by their mental illness symptoms. This is a voluntary court-supervised treatment program in lieu of traditional penalties. The program is a collaborative effort that includes the Portsmouth, Exeter, and Hampton District Courts, local law enforcement agencies, Public Defenders, and SMHC. The goal is three-fold: recovery, savings, and social benefits.

    NH Military Supports

    We participate in a statewide Military Supports initiative, designed to improve access to and the quality of care for veterans, service members, and military families. Our goal is to be sure that our staff better understands the military culture for both the veteran or the service member and their family. We have the ability to provide evidence-based practices when treating combat trauma and we are knowledgeable of the services and resources available to our service men and women and their families.

  • To support comprehensive care, we offer a full assessment of you and your family’s needs. Areas assessed may include housing, education, recreation, medical, legal, food security, benefits, in addition to other basic needs. You and a case manager will develop a comprehensive care plan to address the identified needs. Your case manager will work with you on an ongoing basis to execute your plan and provide follow-up and modification, as needed. The goal of our team approach is to empower you to ultimately reach your identified goals.
